Samsung HeartWise is a home-based cardiac rehab program, allowing doctors to set individual target goals for patients to get them back on track to a healthier lifestyle. In conjunction with the Samsung Gear S3 watch, the smartphone app shows the patient’s daily, weekly and overall progress of exercise, as well as health data in a clear and easy-to-grasp manner. Patients can also set up medication reminders on their watch. All information is automatically sent to the doctor, ensuring a continuous data collection and patient monitoring.
